It's a circular yoke with the dress gathered on, and then the puff sleeves. Other than being REALLY bad at inset sleeves, I could put together a pattern for you.

Inset sleeves are the same as those on a peasant blouse, right?

Probably so. For poofy sleeves, I'd probably forgo a pattern and just gather a long piece of material onto the armhole. :gig

eta: looking at it, it's possible it's done as a t-tunic and the it's all gathered to the yoke and just sewn right up the side seams, sleeves included.

I use that same Simplicity pattern. My daughter loves to wear dresses, but last year (1st grade) some kids pulled her dress up and she was very traumatized. She is extremely modest. So I used the shorty pajama bottom pattern to make her a bunch of cotton "bloomers" to wear under her dresses. She loves them. I made several colors to match a variety of dresses and put cute lace/trim around the leg edges.
